Can AI Replace Therapy? (And the Risks of Using AI for Mental Health Support)

We live in a time where a chatbot can write a resume, generate a meal plan, or even mimic a poem about heartbreak. So, it’s no surprise that AI is now entering the therapy space offering “emotional support,” self-help tips, and even promising to replicate the therapy experience.


Let me be clear as a therapist and research-informed clinician: AI can support mental wellness—but it cannot replace real therapy.


And trying to use it that way can come with some serious risks.


Your Brain Was Built for Human Connection (Can AI Replace Therapy?)


Let’s start with the science. Human beings are wired to heal in connection with other human beings.


From a neuroscience perspective, therapy works because of co-regulation—a process where your nervous system learns to calm itself through safe, attuned interactions with another person. Mirror Neurons are powerful. This isn’t just a nice idea—it’s backed by brain imaging and polyvagal theory.

🧠 Neurofact: When we feel safe with another person, our brain activates the ventral vagal pathway, which down-regulates the stress response (amygdala) and allows access to memory, emotion regulation, and reflection (medial prefrontal cortex). – Porges, 2011; Cozolino, 2017

AI cannot do this. It doesn’t have a nervous system. It can’t mirror yours. It can’t co-regulate. It can only pretend to care—and your brain, whether consciously or not, knows the difference.


Example: What Therapy Does That AI Can't


Let’s say you’re talking about a conflict with your partner. A chatbot might validate you and offer a communication tip.


But a real therapist might pause, notice that you just crossed your arms, your voice went flat, and say:

“You went quiet just now. I wonder if something about this feels familiar—like you’ve been here before?”

And suddenly, the conversation shifts from surface-level conflict to a deep core wound: a childhood pattern of emotional neglect that still plays out in adult relationships.


That’s not just support—that’s relational healing, and AI simply isn’t capable of this kind of nuanced, embodied noticing.


Real Risks of AI in Mental Health Care


AI therapy apps and bots are everywhere. Some are well-meaning. Some are wildly overpromising. Most carry risks no one’s really talking about.


Here’s what I worry about as a clinician:


1. Missed Risk and Unsafe Advice


A peer-reviewed 2023 study published in JAMA Network Open tested AI chatbots' ability to detect signs of suicidality. Most failed.


One even responded with:

“That sounds hard. Have you tried going for a walk?”to a user who said they were thinking of ending their life.

No triage. No referral. No safety planning. Just platitudes.


2. Therapy Is Relationship, Not Just Talk


AI can repeat affirmations. But it doesn’t know you. It doesn’t remember your trauma history, notice patterns, or hold complexity.

True therapy involves rupture and repair—something only a human can navigate ethically and safely.

3. Lack of Regulation or Clinical Oversight


Most AI apps are not bound by provincial or federal privacy laws. Your data might be used to “improve the model,” but what does that mean? Where is it stored? Who owns your pain?


A real therapist is bound by regulatory bodies, ethical codes, and privacy laws (like PHIPA in Ontario or HIPAA in the U.S.). You have recourse if something goes wrong. With AI? You don’t.


When AI Can Support (But Not Replace) Mental Health


I’m not anti-tech. AI can be helpful and used as assistive technology:


  • Journaling prompts

  • Symptom tracking

  • Guided meditations

  • Mood check-ins

  • Between-session reminders


But these tools are like a fitness tracker—not a personal trainer. They support the work. They don’t do the work.


Healing Is Human


So, no—AI isn’t taking our jobs. It’s just reminding us of what therapy really is: an intentional, safe relationship where someone gets to show up as themselves—messy, anxious, grieving, tired, stuck—and not be judged or fixed.


Real therapy is a space where healing happens not because of perfect words or clever advice, but because someone is there, fully present, helping you walk through the hard stuff without turning away.


That kind of safety can’t be scripted.And it sure can’t be automated.
